Un-enrollment Options:

1). 30-Day Cancellation: cancellation is effective 30 days after written notice is provided by completing this form. Client is responsible for tuition until cancellation is effective, even if student is unable to attend class.

2). Expedited Cancellation: cancellation will be effective at the end of current month. Tuition already processed will not be refunded; however, cancellation will be effective before next billing cycle. The $30.00 fee assessed gives the customer the option of opting out of the 30- day notice.
